How to Create Custom Sign-Up Forms for Membership Plans?

This article explains how to set up the new Sign-Up Form, which will replace the old Application Forms.

These updated forms allow you to customize onboarding per membership plan, including support for Group Memberships, Sub-Member fields, and Auto Approval.

What You Can Do with the New Sign-Up Forms

  1. Customize fields for each membership plan

  2. Ask separate questions for sub-members (if Group Membership is active)

  3. Automatically approve applications for selected plans

  4. Collect more relevant and targeted data during the sign-up process

How to Set It Up

  1. Go to your Admin Panel → Membership → Plans

  2. Create or edit a membership plan

  3. Scroll to the Sign-Up Form section

  4. Toggle Enable Sign-Up Form

  5. Add your custom fields for:

    • Primary Members

    • Sub-Members (only visible if Group Membership is enabled)

  • Toggle Auto Approval if you want members to be approved instantly

When to Use Auto Approval

Enable Auto Approval if:

  • You trust the segment (e.g. internal staff, known organizations)

  • You want to streamline onboarding

  • You've added fields to capture all the needed information

New members (and their sub-members) will be approved immediately after submitting the form.

Suggestions for Setup

  • Use clear field labels like “Child’s Age” or “Department”

  • Only ask what’s necessary; keep it short and purposeful

  • Match the tone and language to the audience (corporate vs. family)

  • Use clear plan names like “Family Plan – Up to 5 Members” or “Corporate Membership – 10 Seats”

Migrating from Old Application Forms

If you previously used the global application form:

  • You can now manage sign-up fields directly within each membership plan

  • Sub-member onboarding is now supported via Group Memberships + Form Fields

  • You no longer need to use the “Application Form” settings under the Settings tab
